Visa Assessment Form Download

Thank you for purchasing your Visa Assessment from BobinOz. Please retain your PayPal Transaction ID, you may need to provide it to us if we cannot find your payment in our system.

You can download the forms you require from this page. Please be aware that the Client Information Questionnaire (CIQ) that you need to complete will not work on iPad, iPhone, or Android and it does not work on Mac without a small adjustment, although the Completion Guide PDF will open in those devices.

Please read that Completion Guide in full for further information about that along with other useful tips to help you successfully return your completed questionnaire.

To Download the Assessment Forms:

Please click on each of the following two links to download the PDFs individually.

Important: these PDFs need to be downloaded to your computer and clicking these links should automatically do that, or give you the option to Download or Save. Do not try to open the PDF Questionnaire within your browser, as it may not open but just keep “Waiting…”; more information about this dynamic PDF below.

Important Notes:

The following advice also appears in the ‘Completion Guide’ PDF, but they are worth mentioning here also. IPad and smart device users should certainly read Note 3.

This Questionnaire should only be opened in Adobe Reader (version 8.1 or higher) or Adobe Acrobat (version 8.1 or higher). Other Adobe-replacement PDF viewing software (e.g. ‘Fox-It’) will not work properly, which may result in errors when using/saving the Questionnaire and should not be used. If you are having trouble viewing the Questionnaire, you may need to download an updated version of Adobe reader. This can be obtained without charge from the Adobe website.

To download Adobe Reader:

If you haven’t already got it, you can get a copy of the latest Adobe reader here. It’s free and you can download it by clicking the PDF icon below…..

Adobe

Note 1: Some versions of Adobe Acrobat/Reader give you the option to ‘Save a Copy’. DO NOT DO THIS, as it will remove certain functionality from within the form. If you need to save this form to another location after opening it then you must use ‘Save As…’ in Adobe Acrobat/Reader’s file menu; do not use Adobe’s ‘Save a Copy’ feature, as this will remove certain functionality from the Questionnaire.

Note 2: If you are using a Mac computer, please note that Mac’s ‘Preview’ program is incapable of handling Dynamic PDF documents such as this questionnaire.

Note 3: This is a dynamic PDF document and as such it does not work with iPad or other smart devices without an appropriate app installed. Please search for ‘dynamic’, ‘fillable’ or ‘interactive’ PDF apps for your specific device to find a solution.

Any problems, please do contact me via bob@bobinoz.com

I wish you well with your application and hope to see you here in Australia one day soon.

Many thanks

Bob

I only have an iPad ?. Never mind I guess I will have to wait a few days and use work pc. Looking forward to finding out if my dream move is actually possible.

    • BobinOz August 10, 2018, 7:14 pm |

Yes, sorry about that, it just doesn’t work on iPad. PC is much better. Good luck with your assessment, and I hope your dream does come true 🙂

Thanks Bob, it’s already been a long process. I have topped up my nursing diploma to a degree and got max points for my IELTS. Unfortunately at 43 however, I’m no spring chicken. Like you I’m hoping to move from Essex to Brisbane. Fingers crossed.

        • BobinOz August 13, 2018, 7:01 pm |

Well done on the IELTS, that’s not easy to do. I have my fingers crossed for you as well. Good luck, Bob

      Hi Sylvia

I have noticed that you have also sent me a direct email about this problem, so I am now going to reply to you directly right now so that we can get this fixed.

  • Emily Clayton January 21, 2017, 5:02 pm |

    Hi there,
I’ve just completed the client information questionnaire and because I was using the work computer, it automatically sent it through the work email (Oasis hotel). However, I then re-sent it straight away from my private email address. I just wanted to confirm to not reply to the work email address please, just my private one. Thank you – I hope it worked?

    • BobinOz January 21, 2017, 10:02 pm |

      Hi Emily

I’ve just spoken to my agent and he has not received your completed questionnaire at all, so I don’t think you have successfully sent them from either address. When you click on submit in the questionnaire it automatically attaches it to the default email client on the PC and also completes the email address that we want you to send it to, but you do then have to navigate to the email and additionally hit the send button.

If you check in your sent items, my guess is that you haven’t sent anything to my agent from either address. The best thing is to save your completed questionnaire to your desktop and attach it to your email in the usual way, and then send it to the email address that appears at the bottom of the CIQ in the submit button area.

Hope that makes sense, cheers, Bob

      • Emily Clayton January 24, 2017, 4:44 pm |

Hi Bob, thanks for your help!
I’ve just had time to re-complete it (I stupidly didn’t save it previously because of it being on a work computer.

I’ve done as you said, saved and attached it separately, so hopefully it’s gone through now.

        Thanks, Emily

        • BobinOz January 25, 2017, 12:21 am |

Yes, it’s worked, your completed questionnaire came through earlier today. You should be hearing from my MARA agent very soon. Good luck, I hope it goes well.

          Thanks, Bob

    Hi Bob,

Any help or Advice on how i can open this will be greatly appreciated.

    Kind regards,
    Liam.

    • BobinOz November 23, 2016, 2:23 pm |

      Hi Liam

Sorry to hear you are having this problem as well, it does happen from time to time but is still relatively rare. I have also received an email from you about this and I will reply to you directly now.

It will detail a procedure you can follow to troubleshoot this which has, for other people so far to date, always managed to fix the problem. So check your inbox, you should have the email within five minutes.

      Cheers, Bob

      Hi

I notice you have purchased a visa assessment from this page. What you need to do is complete the PDF questionnaire you will have received and return it to my MARA agent.

He will then look at your details and provide you with your report. If you have any questions after that you can discuss them direct with him during your Skype/telephone conversation follow-up. Good luck, Bob

  • Paul March 6, 2016, 6:12 pm |

    Hi bob
I have purchased your assement from and I can not open it I’ve tried everything!! Please help!!!!!!!
    Kind regards
    Paul

    • BobinOz March 7, 2016, 12:08 am |

Hi Paul, if you are on a PC this is extremely unusual, but if you are on a Mac, iPad or other smart device like Android, then the interactive PDF will not work on those devices without the right app.

That is all explained above, but what I will do now is send you a personal email with further instructions so hopefully you will be able to open the PDFs.

      Cheers, Bob

      • Ajith George March 12, 2017, 5:35 pm |

        Hi Bob,

For using it on a mac, you have to download the adobe reader and open the CIQ pdf using this adobe app rather than the native preview app of apple. That is it. It works very well.

        Best Regards

        • BobinOz March 13, 2017, 8:36 pm |

Sounds easier than watching that video, thanks for the tip. I have added a link to the download page for the latest Adobe Reader for Mac in the the page above.

  • Julius March 3, 2016, 12:09 am |

    Hi
am Julius from Uganda, East Africa, with a valid passport, I wish to get information from u, I need to do a temporary work for six month as either a skilled painter or skilled mechanical maintenance worker in Australia, and therefore, I wish to know if u can get for me any info which can lead me a job placement that can sponsor for my visa, air ticket, relocation and accommodation in Australia , if its possible, what are the requirements do I need to have? Thank u

    • BobinOz March 3, 2016, 8:11 pm |

No, I can’t help you with that, and I think it is highly unlikely anybody would sponsor you in this way and pay all of the expenses you want them to pay. Good luck though, Bob

Hi Bob, Ive just purchased the visa assessment but when i open in adobe reader it is saying the form is secured and will not let me sign. I have the newest version of Adobe. Not sure what to do? Can you help?

    Many Thanks

    Natalie

    • BobinOz February 23, 2016, 3:14 pm |

      Hi Natalie

I will send you an email direct with the PDFs attached, but please do be aware that the questionnaire PDF, which is dynamic, really works best on a PC.

Please read the notes above with regard to Mac and iPad.

I’m going to resend you the documents now, please do let me know how you get on with it. Cheers, Bob

  • Kayleigh February 7, 2016, 2:19 am |

Ive just paid for the forms but I’ve already misplaced the claim number, how do I find out what it is?

    • BobinOz February 8, 2016, 4:01 pm |

Hi Kayleigh, I’m not sure what you mean by ‘claim number’, all I can think of is the first question on the form asks…

      Client File Number
If you have been given a client file number by your agent, record it here:

You don’t have one yet so you can just leave it blank. Is that the number you mean?

      • Kayleigh February 8, 2016, 7:55 pm |

Yes that’s the one thank you, I thought I was given a number when I paid for them but I’m obviously mistaken. I will leave just leave it blank. Thank you for your reply

        • BobinOz February 9, 2016, 8:12 pm |

Great, thanks Kayleigh, and good luck with your assessment.

  • Pankaj January 24, 2016, 9:30 pm |

    Hi Bob,

I downloaded and filled all the required information but not able to submit the form using the Submit button at the end of the form. Some OAuth version error I received from gmail.

Can you please tell me on which email id I can send the filled CIQ form as I didn’t find any email address?

    Thanks,
    Pankaj

    • BobinOz January 25, 2016, 8:55 pm |

      Hi Pankaj

Sorry you have had problems with this, but I have been informed that you have successfully now returned your completed questionnaire and my MARA agent has confirmed to me he has received it.

So it looks like you sorted it before I got the chance to answer your comment.

Good luck with your assessment, I hope it goes well, Bob

  • Kevin December 13, 2015, 12:16 am |

Hi Bob, I’ve purchased the form however I cannot get the form to open on my android advice. I haven’t received a link through email. Is it possible for you to send me a link via email and I’ll open it on a computer. Thanks.

    Kevin

    • BobinOz December 13, 2015, 12:47 am |

      Hi Kevin

No, it doesn’t work on Android, it’s best to use a PC or a Mac. I’ve already sent you the forms that you need, you should have them by now. If you can’t see anything, just check your spam or junk folders. Do get back to me if you still have problems.

      Cheers, Bob

  • Silvina Samy September 16, 2015, 5:18 am |

    Hi Bob,

I purchased the assessment forms a little while ago and already submitted the filled questionnaire and CV via email. I hope you’ll be able to shed some light in this migration process… Will I receive a follow-up email?

    Thanks in advance.

    Kind regards,

    Silvina Samy.

    • BobinOz September 16, 2015, 11:50 pm |

Yes, you will. In fact I spoke to my MARA agent earlier today, he tells me he has already acknowledged receipt of your questionnaire and CV, so you can expect your visa assessment shortly.

And for sure my agent will shed some light on this process for you, so good luck Silvina, I hope it goes well for you.

      Cheers, Bob

  • Kashif July 12, 2015, 2:42 pm |

    Hi Bob,

For a moment i felt worried as after payment, i only saw a blank page but thanks for sending link . I’ve even completed and reverted back including CV to Simon. Hope so it will work and help to find a gateway to move on till i achieve my goal.

    regards,
    Kashif

    • BobinOz July 12, 2015, 9:25 pm |

Yes, my MARA agent copied me in on the reply to your email about not receiving the forms, not long after that my agent informed me that he had received your completed questionnaire, so I was very pleased to hear that it had all been sorted out.

For some reason, sometimes the page doesn’t automatically redirect as it should, that’s why I send out the follow-up email. Good luck with your assessment, I hope it goes well.

      Cheers, Bob

  • Lilly April 30, 2015, 8:50 pm |

    Hi Bob,
I’ve just purchased your assessment and holding thumbs that it gives me a bit of direction.
We are not sure whether it would be “easier” to get in on my “Skills” or if we should rather do it on my husbands name.
So my question is actually this, can I complete the assessment twice and submit both or do I need to purchase the assessment again??
And also have you dealt with anyone in South Africa trying to Migrate? and what is your success ratio?

    Kind Regards,
    Lilly

    • BobinOz May 1, 2015, 6:41 pm |

      Hi Lilly

Thanks for purchasing an assessment, I’m sure it will point you in the right direction.

The fee only covers one assessment report for one of you, but at this stage it is not necessary to purchase a second assessment.

Try to decide which of the two of you is going to be the main applicant and hopefully you will know which of you has the most desired skill and therefore the best chance of qualifying. The questionnaire has some space on it so you can provide some details of the secondary applicant on the form.

Only one of you needs to qualify and the spouse automatically qualifies with them.

In your covering email when you return the form, mention to my agent that you were not sure which of the two of you should be the main applicant and suggest that he is welcome to switch to the other person if he felt it would be best. If my agent feels that the secondary applicant should actually be the main applicant, he will not go ahead with the full assessment, but instead return the forms to you and request that you re-complete them with the new main applicant details.

If you both have a good chance of qualifying though, there may well be some advantage in you both lodging if you are going through EOI, it will double your chances. But that’s something to discuss with my agent.

Hope that helps and good luck, Bob

  • Nina Woodward March 2, 2015, 12:04 am |

    Hi bob,

I have just purchased the assessment but could not open on my pad. Could you please send me in email.

Looking forward to completing and sending back.

    Nina

    • BobinOz March 2, 2015, 8:07 pm |

I understand from my MARA agent that you returned your completed questionnaire today Nina, so it sounds as though you’ve sorted it all out. Good luck with your assessment, I hope it goes well.

      Cheers, Bob

  • Fiona Luke January 8, 2015, 2:13 pm |

    Hi Bob,

My partner and I purchased the assessment forms a little while ago and have just got round to filling them out. I have come across the part that ask’s if you have previously applied for an Australian visa. We have filled in the details of both of the 417 working holiday visa’s that my partner has applied for, but it ask’s for both a visa number and a visa grant number. On the emails received regarding both 417 visas it only states the visa grant number? Do you know where I might find the other visa number that apparently starts with a ‘V’? The form gives the example: V807<3284575M

    Thank you for your help.

    Kind regards

    Fiona

    • BobinOz January 9, 2015, 1:39 am |

      Hi Fiona

Thanks for purchasing an assessment, I hope it goes well for you. As for those specific numbers, I really would not worry about it at this stage. Just fill out the forms as much as you can, these specific details aren’t overly important at this stage.

My agent will be able to do a full assessment without them, if you then go on to lodge an official application then may be those numbers will be required, but I’m sure he will be able to help you to get them.

Just send in what you can and if he needs anything extra, he will ask you. This isn’t an official application at this stage, it’s just my agent gathering information about you.

Hope that helps and good luck, Bob

  • Rahul December 5, 2014, 10:50 pm |

    Hi Bob ,

Just purchased the assessment form today, my agent in India was charging me around 3 times the charge. Got a great deal from you ! Thank you ! hopefully i fill in everything and get the process started quickly. Just a small query, do the MARA agents work through out the year? are there any breaks, as i expect to complete this questionnaire only in the next 15 days and that would be the start of christmas break for you guys.

    Thanks a ton again!

    • BobinOz December 5, 2014, 11:53 pm |

      Hi Rahul

Glad to hear you have purchased an assessment and I’m also please that you have saved some money. Excellent!

Yes, I’m sure my agent will be taking a break for Christmas, I’m not sure yet of the exact dates. If it really is going to take you 15 days before you return it, then it is very unlikely it will be done before he breaks for Christmas.

I will try and find out what his plans are for Christmas and I’ll post something on the main Visa Assessment Service page.

It would be much better though if you could return that form within five days 🙂

Good luck with your assessment, Bob

  • Nitin June 18, 2014, 2:21 am |

Hi! Posting the query again.. do we need to fill the one pager CIQ with the 6 pager CIQ in case we intend to opt for the general assessment. Thanks – Nitin

    • BobinOz June 18, 2014, 2:31 am |

It’s all fully explained fully in the PDF within your download zip, but if you just want a general assessment and you have no specific questions, fill in the six page CIQ.

      Cheers, Bob

  • Nitin June 18, 2014, 2:18 am |

Hi Bob, Do we need to fill the one page or 6 page CIQ in case we intend to opt for the general assessment.

  • Priyanka June 18, 2014, 1:35 am |

Hi Bob, we’ve just purchased your visa assessment services. Hope this will help us in settling in Australia soon. 🙂

    • BobinOz June 18, 2014, 1:27 pm |

      Hi Priyanka

      I hope it does too.

Good luck with your assessment, I hope it goes well.

  • Liz March 27, 2014, 3:32 pm |

    Hi Bob,

I have purchase a visa assessment form and in filling it out I noticed you said to provide and up-to-date CV. Is that the info asked for on the form or does that need to be a separate document attached?

Also, I have a boyfriend and we have been together for two years, but cause we are young we don’t live together. He is an Australian citizen. I researched that a de facto relationship meant you had to reside together so I wanted to see how/if/where I should put his name and details down on the form.

    • BobinOz March 28, 2014, 1:13 am |

      Hi Liz

The CV, yes, that’s a separate document to attach but only if you are looking for a skills-based visa, sounds to me by what you say that that isn’t the case for yourself and your boyfriend.

You are correct, a de facto relationship requires that you have lived together, specifically for at least 12 months. As you haven’t, and as your application may not be skills-based, then I think you need to be asking my MARA agent how best you can proceed to be able to lodge an application.

I would complete the form in your name with your boyfriend as your partner but explain that you do not live together in a covering email to my MARA agent and he will advise how best to proceed from that.

Good luck, I hope it works out.

      Bob

  • John Rash February 2, 2014, 2:34 am |

Thanks for your service, I was able to buy an assessment for myself, but when tried (3X) to have a separate one ordered for my wife on same card (wife is RN), it would not let me, Please advise.

    Thanks!

    • BobinOz February 3, 2014, 1:18 pm |

      Hi John

Sorry to hear about the problem you had purchasing a second assessment, not sure why it has happened, it may be some kind of built in security feature from PayPal.

Probably a good thing it happened, I don’t think you need to buy one for each of you. May I suggest you use the visa assessment you have purchased to complete the details of which ever of the two of you would think it has the best chance, in your opinion.

You only really need one main applicant and if that applicant is successful, then the other qualifies automatically as a spouse. If my agent thinks the main applicant should be changed for some reason, then it can easily be changed.

      Good luck, Bob

  • Jo November 30, 2013, 5:21 am |

    Hi Bob

Love the website – lots of useful information.

I’ve started filling in the visa assessment form and I am not sure how much detail to include.

Re education – do I just include my accountancy qualifications or does he need all ‘O’ and ‘A’ levels etc.?

Re travel in the last 10 years – I have been lucky enough to travel to Portugal once or twice a year as well as some other trips elsewhere and it would take a lot of research to check all the dates, even if it is possible. Does he really need all that?

Re previous visas – I had a holiday in Australia about 20 years ago but I don’t know the exact dates or the details of the visa. Is this important?

Thanks for any help you can provide.

    Regards

    Jo

    • BobinOz December 1, 2013, 7:49 pm |

Hi Jo and thanks, glad you like my website.

As for the forms, just do the best you can with it and for those trips abroad where you don’t know the dates, there’s no need to worry about that from now. Example, just put something like “visited Portugal around 15 times over the last 10 years, holidays only, dates unknown”.

For education, I’d certainly put down all your professional qualifications and if you can, your “A” and “O” levels.

If you have a few blanks on the form that you can’t fill in, don’t worry for now, just explain them in a covering email when you return the form.

Good luck with your assessment, I hope it goes well.

      Cheers, Bob

      • Jo December 2, 2013, 11:13 pm |

        Thanks Bob.

I’m in the very early stages of thinking about making the move over to Australia having had an extended holiday there recently and loving it.

The reason for requesting the assessment is initially to be steered in the right direction and get a general idea of my options and likelihood of getting a visa so I didn’t want to be spending days trawling through diaries and emails trying to piece it all together.

        Thanks for your help.

        Jo

        • BobinOz December 3, 2013, 12:20 am |

          Hi Jo

I spoke to my MARA agent today, he actually said not to worry too much about all the foreign travel, just be sure to mention the countries you visited for longer periods, say three or four months or more.

This assessment will certainly steer you in the right direction, whatever options you might have you will be given. It will save you endless research.

          Cheers, Bob

  • Laurette October 19, 2013, 3:07 am |

    Hi Bob,

We are having the same problem as a few others who have tried downloading
the form to our iPad. Can you please send it to our email? Thank you

    Laurette

    • BobinOz October 19, 2013, 7:50 pm |

      Hi Laurette

Yes, I’m afraid this PDF doesn’t work with iPad because it is a dynamic PDF, I believe it presents you with questions to answer based on previous answers, so it reacts to the information you provide.

I have now added that information in the notes about the download on the Visa Assessment service page and above.

My automated email system did send you the documentation after you signed up, hopefully you have received that by now or, if not, it will almost certainly be in your spam or junk folder. No worries though, I am now going to send you the forms again direct to your email right now.

      Good luck with your assessment.

      Hi Gemma

I have now sent you the forms you need, I’m also looking into how this document can be downloaded using an iPad. I’ll update the information above when I find out how it works, assuming it does somehow.

Good luck with your assessment, hope to see you here in Australia soon.

    Hi Bob,

I am currently filling out the Visa Assessment for Miki. I have a query, it is asking for every address I have lived at within the past 10 years. Does this include term time address whilst at university? I still lived at my current home address out of term (e.g Xmas hols and summer hols, odd weekend)
My driving license and all other important documents have only ever been registered to my current home address. If you could clarify it would be greatly appreciated.

    All the best

    James

      Hi James

I seriously doubt that you would need to fill in all of these addresses, but just to be sure I’m going to check with my MARA agent and either he or I will come back to you.

      Speak soon

      Bob

        Hi James

I couldn’t get hold of Miki today, I’ve been told he has been in meetings all day. If you are in a hurry to return your form, it might be a good idea to do so without all those extra addresses as I’m sure they won’t be needed, but if they are Miki will simply ask you for them.

        Cheers, Bob

    Hi Bob,
We’ve just downloaded the application forms for Miki and will be filling them in and sending them off this week. Thanks for the link, we talked to our friends in Oz and they recommended we go with your’ agent as it was better to deal with an agent in Oz.
    All the best,
    Lee jobson

      Hi Lee

Yes, I did see today that you have booked your assessment, I hope it goes well for you and you have plenty of options. Miki will certainly look after you, he is very good and as soon as he has your assessment through you should hear back from him within three working days or so.

Good luck, hope to see you here soon

      Hi Lou

Yes, I saw your purchase go through earlier today, hope the form filling is going well.

There is no time limit, so take all the time you need, although do bear in mind some of the rules may change at the beginning of next month, although there is nothing much we can do about that as it is almost certainly too late to lodge a full application before then.

      Many thanks

      Bob

Hi bob I have purchased the visa assessment doc via my PayPal acc but cannot download the form on my iPad. Could you please and me the details to my email address please thanks Nicola pollitt

    • BobinOz January 18, 2013, 1:56 pm |

      Hi Nicola

Sorry you couldn’t download the form on your iPad, I have now sent it direct to your email account as requested. I notice you use Hotmail, if you can’t see anything in your mail folder, please check your junk/spam folder as Hotmail has a habit of putting legitimate email into those folders.

      Good luck with your assessment!

      Bob

    Hi Hemang

Yes, I did see that you purchased a visa assessment, have you completed the form that you downloaded from this page and returned it to Miki? If so, there’s nothing more for you to do right now, Miki will do the assessment within the next few days and get back to you directly.

If you have any other questions, you can also email Miki direct on the same email address supplied to you.

Good luck with your assessment, I hope it goes well.

    Cheers

    Bob

My Form just vanished off screen. I think some of the questions will not be relavent as I wish to get a Return Residents Visa 155. Could you please advise as to what I should do next.As you know I have paid my fee what will happen now?
        Kind Regards
        Alex

        • BobinOz March 2, 2016, 4:47 pm |

          Hi Alex

That’s strange, but I have no idea why it vanished off of your screen, must be something your end though. I suggest you download it again if you can’t find it on your computer, you can just click on the download links above to do that.

The questionnaire is also interactive, so certain relevant questions pop up next dependent upon how you answer previous questions, so this questionnaire is relevant for you.

So if you can give it another go, that would be good. Let me know if you have any further problems. Cheers, Bob
